The intention of the LifeTime Health Center clinic is to provide high quality, expert health care at an affordable cost to patients.
The LifeTime Health Center clinic offers family practice services, handles minor emergencies and specializes in hormone replacement therapy, and weight loss. Lab fees are very reasonable. Other services include care for chronic conditions (high blood pressure, diabetes, hypothyroidism), lab work, electrocardiogram(EKG), colposcopy, family planning and contraception. Services in this office are provided by certified, licensed Physician Assistants and Nurse Practitioners. If you wish to see a physician or your condition requires specialty care, you will be referred to one.
Office Visit for New and Returning Patients - $60
Physical exam packages include the following:
Complete Female Exam - $175
Physical and breast exam, health risk history, complete metabolic panel and thyroid stimulating hormone, blood count, Pap smear, and cholesterol panel.
Complete Male Exam - $175
Physical exam and health risk history, cholesterol panel, complete blood count, complete metabolic profile, Thyroid stimulating hormone, Testosterone, Prostate specific antigen and (EKG)
Complete Hormone Evaluation
Premenopausal Women - $175
- Includes office visit, Hormone Evaluation questionnaire, evaluation of any existing labs, recommendations for testing and TFTs (thyroid tests)
- SALIVA TESTING*; test costs are separate from visit and range from $150 to $200.
- Other lab fee's are seperate from office visit.
- Follow up Office Visits are $60, and are necessary to receive results and treatment plan.
Post Menopausal, Surgical Menopause or any woman on HRT - $200
- Includes all in the Complete Hormone Evaluation
- Serum Blood Labs in office $200
- Includes follow up Office Visit or phone consultation to review results and treatment plan. Patient should schedule at the conclusion of the Hormone Evaluation for 2-3 days later.
- Blood testing for women includes serum estradiol, progesterone, DHEA, testosterone, SHBG, Vitamin D, Thyroid functions (TSH, T3)
Male Hormone Evaluation - $175
- Includes Office Visit, Male Hormone Evaluation questionnaire, evaluation of any existing labs and recommendations for testing.
- Labs in office are seperate fees, ranging from $125 to $200*
- Includes follow up Office Visit or phone consultation to review results and treatment plan. Patient should schedule at the conclusion of the Male Hormone Evaluation for 2-3 days later.
- Blood testing for men includes tests for testosterone, SHBG, DHEA, PSA, Vitamin D, Thyroid functions (TSH, T3)
(*)Saliva test cost is paid at the office. The kit is sent home with the patient with instructions for use.
Testing for women or men already on hormones should be done as close to 8 a.m. as possible. Fasting is NOT required for Hormone Evaluations.
Payment in full is expected at the time of service. LifeTime Health Center does not accept any insurance as payment, including Medicare, Medicaid or Workers Compensation. We accept cash, checks and credit cards. We will give you the estimated cost of any service prior to providing the service.
Last-minute cancellations:
Because LifeTime Health Center provides such affordable healthcare to our patients, our schedule is vital to our mission. Patients scheduled for a 30 minute appointment, such as Hormone Evaluations, who do not provide cancellation notice in advance will need to make a pre-paid $50, non-refundable deposit for their next appointment. This amount will go toward the next appointment.
